Conference #3 Integrated care projects for chronically ill patients
On March 14, 2023, the Sustainability Unit of the Faculty of Medicine at the University of Liège and Eclosio organized their third conference on the theme of"Integrated care projects for the chronically ill"
The integrated care is care in which health services are organized in such a way that individuals experience continuity of care within the whole of its services, both in terms of :
- Health promotion
- Prevention
- Diagnosis
- Treatment
- Disease management
- Recovery and palliative services
This approach therefore goes beyond the healthcare sector, as it is based on the principles of population management and coordinated at different levels. What's important here is that the individual is involved as a partner in the care process, and that this care is aligned with his or her biopsychosocial needs.
Currently, 12 pilot projects (spread across Belgium) are testing initiatives aimed at implementing integrated care primarily for chronic patients, with a view to extending them to the general population.
